I certify that the information provided on this Application is true and gives the Company, its subsidiaries, and its agents the right to investigate all references and information given by me in this Application. I hereby consent to an examination, including screening for drugs and/or chemical intoxicants. I understand that if I test positive for drugs or refuse to be tested, I will be removed from further consideration at this time. I hereby release all claims against the Company, its team members, and its agents, and any person supplying information to them. I certify that I received written notification that this Company may obtain consumer reports on me, and I authorize this Company to obtain such reports in connection with my application for employment and for other employment-related reasons. If hired, this authorization will remain on file as on-going authorization for the procurement of employment-related consumer reports at any time during my employment. I understand that the term “consumer report” includes, but is not limited to credit checks, criminal background checks, Motor Vehicle Records (MVR) reports, and investigative consumer reports. I further understand that the term “investigative consumer reports” means a report in which information on my character, general reputation, personal characteristics, or mode of living is obtained through personal interviews with my neighbors, friends, or associates, or others with whom I am acquainted or who may have knowledge concerning any such information. I understand that false statements or omissions of facts are cause for refusal to hire or immediate dismissal. I understand that my employment with the Company does not constitute a contractual relationship and may be terminated according to local, state, and federal laws. The Company is liable only for wages or salary earned up through the date of such termination. I further understand that I have the right to terminate my employment at any time, with or without notice or reason, and that the Company has the same right. No one other than the CEO of White Hat Corporation has the authority to modify this relationship or make any agreement to the contrary. Any such modification or agreement must be in writing. Finally, my signature below authorizes the Company, upon my employment termination, to deduct any debts owed to the Company from my final paycheck. This Application is valid for 60 days.
